Ordering Gift Albums: Step-by-Step

Click on the Order-Form below to download and print.
Using the Order-Form, please follow these easy steps to place your order:

• Step 1: Indicate the quantity needed for each Gift Album Level (by value)
   
• Step 2: Choose one of our themed insert sheets and matching seals, by type:
  • Years of Service - with McDonald's Arches, personalized with employee's name
  • Employee of Month - with McDonald's Arches, personalized with names or not
  • Holiday Greetings - with McDonald's Arches - not personalized
  • Other available "blank" insert sheets are: Thank You, A-Gift-For-You, Congratulations, or Safety. These sheets are normally sent blank, ready for you to add your own message.
     
  • Click to see sample images of each sheet
  
  If you want us to personalize your insert-sheets with each employee's name, please email your list of names in an excel file to ensure accuracy. Please contact us for details.
   
• Step 3: Complete the Order-form information (with your Name, Company, DBA, email, etc)
   
• Step 4: Tell us if you have any special shipping requirements and date needed.
   
• Step 5: Fax your Order-form. Please include the Credit Card form with your first order.

You will receive your order within 5 to 7 business days, or sooner if rush shipment is requested.

Note: When re-ordering and using a previously supplied credit card, you do not need to send the credit card information again.

Simply fill out the forms and Fax them to us.

The free Acrobat Reader is needed to access these forms. Use this link to download the reader if you do not already have it.

• Order Form: Use this Order Form to order Gift Albums.
   
• Paying by Credit Card: Include this form when paying by credit card.
   
• Credit Application: Please use this form if you want us to bill you. A great option when frequently ordering. This form is required with your first order (and only if you are requesting credit). Your information will be held in the strictest confidence.
  Note: You can provide your own Credit Information form if available.
